Specimen Collection and Handling Guidelines

The accuracy of any test procedure is dependent on the quality of the specimen. The quality of the specimen is dependent on how and when it is collected, the care given to its preservation, and how soon it reaches the laboratory.

 

1) Fill in Request Form

  • Patient’s Full Name
  • HKID or Passport No.
  • Sex/ Age/ Date of Birth
  • Referring Doctor’s Full Name
  • Collection Date/ Time
  • Fasting/ Random
  • Cash/ On Accoun
  • Test Requested/ Specimen Type
  • Clinical Information

 

2) SpecimenLabelling 

Each specimen must be labelled with two identification details of the patient.    

      1. Patient’s Full Name

      2. HKID/ Passport No or Patient No./ Clinic No.

 

3) Storage and Transportation

Unless indicated, specimens should be kept in room temperature (Air-condition) and away from sunlight until pickup.Special handling may be required for some specimens, which is specified in later sections.

 

4) Rejection of Unacceptable Specimens Criteria

 

Rationale

This guideline reflects specimen identification and acceptance criteria that comply with ISO 15189 requirements and good clinical practice.Community laboratories have adopted this Guideline so that patients and physicians can be assured that the difference from pre-analytical error is minimized.

 

Rejection Criteria

  • Unlabeled specimens
  • Incorrectly labeled specimens/ request form
  • Insufficient or overfilled specimens
  • Incorrect container or preservative
  • Leaking or broken containers
  • Delayed specimen arrival time
  • Incorrect storage Temperature

 

General Information

1. Specimens will be picked up as soon as possible when the center is informed.

2. Please enquire by telephone if the tests required are not on the list.

3. Off site or home blood-taking service for the convenience of patients may be provided by

    appointment. An additional charge will be levied.

4. Request forms and specimen containers are supplied to clients on request.
